The Young Carers Team supports children and young people who are impacted by the care needs of a family member who may have a physical or learning disability, long term illness, mental ill health or drug/alcohol dependency. The Young Carers Team provide an assessment and support service to young carers, aged 0-18 years, and their families, across West Sussex. The post holder will have a county-wide remit and will be responsible for co-ordinating and supporting the delivery of a range of breaks for young carers primarily in conjunction with the Youth Support and Development Service. This will involve: - • Liaison with staff in relevant services to co-ordinate and support the delivery of holiday activity days and term-time local support groups. • Undertaking responsibility for the mail out of invitations and processing the subsequent bookings for group activities. • Follow up with families where there are additional questions needs or concerns. • Co-ordinating the provision of transport to local groups and working closely with Young Carers Volunteer co-ordinator as part of this provision. • Tracking and monitoring attendance. • Maintaining database of all participation in a range of group activities, individual breaks and commissioned services. • Producing reports You will need to be highly focused, self-motivated and have outstanding organisational skills. You will also need excellent communication skills and an ability to interact sensitively with parents and young people. You will have a relevant qualification and sufficient experience of working in a multi-disciplinary team delivering support to young carers. Some evening and weekend work may be required.
