Age UK West Sussex delivers a range of services and activities to meet older people’s needs. Our aim is to promote the well-being of all older people and make later life a fulfilling and enjoyable experience. We have a range of vacancies within our Information and Advice Team and want to recruit enthusiastic individuals who enjoy working with older people and can support us to continue delivering quality services. We are looking for a special Manager to lead and support our team of 15 dedicated staff and 80 precious volunteers delivering vital services to vulnerable people who are either, in hospital, about to leave hospital, or settling in at home after a stay in hospital. We are searching for an organised person with transferable skills, common sense, flexible approach and a can do ethos with a talent for problem solving. A full driving licence and car would be essential for this role. Our Home from Hospital service provides older and vulnerable people with low level support for a limited period after hospital discharge. The purpose of the service is to ensure people have a successful hospital discharge by accessing help and support and regaining their confidence to ensure they are able to return to independent living. The Take Home and Settle and Relative Support Services provide vulnerable people with support to return home safely rather than be admitted to hospital, once at home we make the person comfortable and safe and refer onto other services for ongoing support. We cover seven hospitals across West Sussex and when not taking people home the staff visit the wards and offer families the Relative Support Service providing information and advice on a range of issues that can arise resulting from patients and families having to make life changing decisions in relation to ongoing care and housing options.
