Going Local is an exciting new project for Adur and Worthing that aims to build and test a completely new way of working between Primary Care (through GPs), the Councils and communities. Referred to elsewhere as ‘social prescribing’, or ‘community connecting’, the purpose behind Going Local is of connecting social and community based help and support - as opposed to just medical responses - to individuals presenting to their GPs with social, emotional or practical needs. This is a shared venture between Adur and Worthing Councils (who will be hosting the posts), West Sussex County Council and the Coastal Clinical Commissioning Group. Designed around the individual - their needs, wants and capabilities - the role of the Community Referrers will be to spend time with people, to deeply explore and uncover the underlying issues and help to navigate, signpost and refer individuals to the right help and support. A combination of values, skills and capabilities are requisites for these posts. We need people with a passion for working with people and making a difference. You must be a skilled communicator - questioning, listening and uncovering issues - strong at prioritising (seeing the wood for the trees), and a tenacious, capable and tactful networker, organiser and challenger of people and agencies. You must be able to skilfully sit in, navigate and be comfortable with different professional worlds and be able to see and hold multiple perspectives. Whilst funding is in place for 2 FTE posts we can see this also working well with part-time posts. Please DO NOT apply via the WSCC website.
