Lead Assessor required for Adults and Children’s Services

Crossroads Care South Central is currently recruiting a Lead Assessor to deliver a robust and efficient assessment process. Managing a team of County Assessors, and ensuring the assessment process is fit for purpose, this post will also be part of the Urgent Response Team and will be an active member within Crossroads Care Duty on Call Team.
The position is full time and will consist of 35hours per week. Based in our office in Climping, we offer an attractive package of £10.50 per hour plus mileage, paid holidays and contributory pension scheme.

PLEASE NOTE THIS IS NOT A WEST SUSSEX COUNTY COUNCIL POST AND IS BEING ADVERTISED ON BEHALF OF CROSSROADS CARE Information sourced during the assessment process will be documented to construct a full and detailed Care Plan and then held within the company system, complying with our systems and processes.
A positive attendance and completion on training will be required with completion and timescales adhered to. A minimum pass rate of 90% is required for all training for this position.
As part of the Duty on Call Team you will be required to receive calls during and outside office hours to support clients and staff with respite or assessments as directed by the Care Co-ordinator. You must be able to travel independently across the county and be part of the out of hours point-of-contact rota.

A level 3 (or equivalent) Diploma in Health & Social Care is a requirement for this position and experience of working in Health & Social Care, Assessing care needs & developing a care plan is also required.
