The team of Take Home and Settle Coordinators provide vulnerable people with the support they need to return home safely rather than be admitted to hospital due to a lack of support at home. They carry out their duties in a Princess Royal hospital, Haywards Heath, East Surrey Hospital in Crawley and St Richards hospital in Chichester. The Co-ordinators are familiar figures in A&E and other hospital departments, they make sure the person is happy to be taken home, ensure it is appropriate and safe for the person to be taken home, once at home make the person comfortable and safe and refer onto other services for ongoing support. While at the hospital during times when not taking people home the co-ordinator visit the wards and offer families the Relative Support Service to provide information and advice on the range of issues that can arise. Our aim is to recruit enthusiastic individuals who enjoy working with people to enable them to take control of their situations and support them to make informed choices. As a bank member of staff you will be asked to cover shifts affected by annual leave or sickness, on an ad hoc basis. You would be an integral part of the team, and this could possibly lead to a permanent post if or when vacancies arise. You need to be an organised person with good administration, observation and communication skills. This post needs you to have a full driving licence and use of a car.

7 hour shifts from 12.30pm to 19.30pm hours seven days a week 9.30 per hour  

The Take home and Settle service is delivered in 7 hour shifts, 7 days a week.
