The principle requirement for the post holder is to assess the needs of the Carer and the person with care needs. Constructing appropriate care plans to support our various respite services including emergency services assessments.

This post holder will also be part of the Urgent Response Team and will be an active member within Crossroads Care Duty on Call Team. This involves receiving calls and responding appropriately to the Care Coordinator request to support the clients and staff of Crossroads Care South Central and our statutory partners. This may result in a waking night or day time package of support.

While at the assessment process the needs of the carer and the person with care needs will be sought and reported accordingly. This includes full risk assessment/medication and any supplementary information as stipulated under Care Quality Commissions requirements.

Information sourced during the assessment process will be documented to construct a full and detailed Care Plan and filed accordingly within the company system while complying with company systems and processes.

The post holder will be part of the Duty on Call Team. This involves receiving calls during and outside of office hours to support clients and staff with respite or assessments as directed by the Care Coordinator. There is an expectation that the hours achieved under this element of roles and responsibility is ten hours per week but on occasions this may be under or over achieved and management of hours will be communicated with their Line Manager.

In liaison with the Care Coordinators/Line Manager, to work closely with local statutory, voluntary and independent organisations to build good relationships and facilitate effective partnerships.

To undertake additional elements relating to this position within an evolving and forward thinking organisation and as directed by the Senior Management Team/Line Manager.

Hours: 35 hrs made up of 25hrs per week assessing and 10hrs per week as urgent response carer support worker.

You will be working from home and reporting into our head office in Climping.
