The Relative Support Co-ordinators work at Bognor War Memorial Hospital and Crawley Hospital providing information and advice on the range of issues that can arise resulting from patients and families having to make life changing decisions in relation to ongoing care and housing options. This would involve taking details of their particular circumstances providing quality information, and if they have complex issues you will be providing a priority referral to Information and Advice for ongoing support. The co-ordinator will have the capacity to deliver face to face appointments with relatives and visit the wards during visiting times (18.30 to 20.00 hours) to ensure people are aware the service is available and how to access it.
