An exciting opportunity has become available for a Senior Social Work Practitioner to work alongside Health colleagues at East Surrey Hospital . This will be an exciting and challenging role.

You will act as a social Work professional responsible for delivering a statuatory social work service to those people in the Northern Area of West Sussex hospitals in order to carry out assessments, planning, implementing and review within an SDS framework.

You will be responsible for leading a complex and challenging portfolio of case work, attending multi disciplinary meetings with the hospital setting and working in partnership with our health and community colleagues. To arrange timely discharges in line with the delayed discharge act.

The impending implementation of the Care Act means that we will be working within a changing and dynamic environment, with the associated challenges and opportunities this will bring.

Salary £33,857 to £37,483
